- What you earn (wages, salary and other) and what you spend
- Details of what you own and what you owe
- Your driver's licence number
- Your ABN, ACN/ARBN and/or Business Registration Number

| After you submit your application |
- You will receive an email confirming the receipt of your application and we will contact you within 48 hours to advise on the progress of your application
- We will post you a Business Purpose Declaration form for you to sign and return to us
