Before we can accept your application, you must acknowledge and agree to the following:

Consumer Credit Insurance

  • I have read and understand the information in the NAB Credit Card Cover PDS (PDF 370kb) under the heading “Important Information About Consumer Credit Insurance”.  I understand that I am not required to buy this insurance.

Issuer

  • I understand that NAB Credit Card Cover is issued by MLC Limited (MLC).  Neither National Australia Bank Limited ABN 12 004 044 937 (NAB), nor any of its related bodies corporate (other than MLC as insurer under the terms of the policy), guarantees or accepts any liability for NAB Credit Card Cover, and a NAB Credit Card Cover policy does not represent a deposit with or other liability of NAB or any of its related bodies corporate (other than MLC as insurer under the terms of the policy).

Exclusions

  • I have read and understand the exclusions that apply to NAB Credit Card Cover as set out in the PDS.  These include that no benefit is payable under the policy for death, critical illness or disability arising from a pre-existing condition (as defined in the PDS).

Premiums

I agree, in respect of each policy I take out:

  • to pay the premium calculated and charged of 79 cents for each $100 (or part thereof) of the closing balance (up to a maximum closing balance of $50,000) owing on my NAB credit card account covered by the policy on the closing date of each statement period while the policy is in force,
  • that the premium is to be automatically charged to my NAB credit card account covered by the policy or any replacement or substituted NAB credit card account/s, and
  • that any future change in premium made under the policy may be charged in the same way.

Privacy

  • I instruct and authorise NAB to provide MLC with any information that NAB holds or will hold about my NAB Credit Card Account contract, and I authorise MLC to seek and obtain access to that information for any purposes associated with the NAB Credit Card Cover policy. This authority overrides any contrary instruction previously given by me to NAB.
