To operate or request changes to your Facility, you will generally need to provide us with written instructions. Download these forms to provide us with instructions that we can action promptly on your behalf.
NAB Equity Lending - Standard forms
| Security/managed fund transfer instruction (232kb) | Use this form to transfer shares and/or managed funds to your NAB Equity Lending Facility or to provide additional security for your loan. If you haven't provided security to NAB Equity Lending to commence trading, you also need to complete this form. |

| Financial adviser/broker authority (227kb) | Use this form to provide NAB Equity Lending with the authority to settle with the broker you will be trading through. |
| Managed fund application/lodgement instruction (226kb) | Use this form to acquire a managed fund investment. |
| Managed fund redemption instruction (226kb) | Use this form to redeem a managed fund investment. |
| Request to increase Facility limit (225kb) | Use this form to request an increase to your Facility limit. |
| Registration for SMS Messaging Service (242kb) | After reading the SMS Messaging Service Terms and Conditions, complete this form to register to receive SMS messages regarding your Facility. |
| Request to increase Facility limit - Guarantor consent (225kb) | If there are any guarantors to your Facility, use this form to obtain the guarantor's consent. This is required before we can approve the increase. |
| Request to establish instalment gearing (229kb) | Use this form if you would like to establish instalment gearing. |
| Request for direct debit (225kb) | Use this form to arrange to pay interest on your margin loan from a nominated bank account. |
| Request for advance (226kb) | To request an advance from your Facility, complete this form. |
| Request to change interest rate option (225kb) | Use this form to request a change to the interest rate option applying to your margin loan or a portion of your loan. |
| Refinance instruction 227kb) | To refinance an existing margin loan that is currently with another margin lender, complete this form. |
| Client representative(s) authority (226kb) | To appoint an authorised representative to operate your Facility on your behalf, complete this form. |
| Change of Name/Address Advice (224kb) | Complete this form to advise us of a change of name/address. |
| Email unsubscribe request (257kb) | Use this form to unsubscribe from receiving emails if your Facility goes into a buffer or margin call position. |
| New guarantor application (139kb) | Complete this form to arrange for a new guarantor to provide third party security for your margin loan. |
