This form lets you tell us when a customer has died. It’s the first step in the process of finalising their affairs with us, and should be submitted by the customer’s next of kin or the estate executor.

We’ll need a certified copy of any one of the following documents:

  • Death certificate
  • Funeral notice
  • Medical certificate confirming date of death

We’ll also need a certified copy of your photo ID, such as a drivers licence or passport.
